We’re excited to welcome new patients to KY Integrative Autism & Family Care! To ensure a smooth and efficient process, all new patients must complete the following steps before an appointment can be scheduled:
Step 1: Submit Basic Info

You may also call our office at 859-592-4992 to provide this information if you prefer to do it over the phone. Submitting online helps us stay ahead of the game and prepare for your care more efficiently.
Step 2: Complete New Patient Paperwork & Pay Fee
Once we receive your basic info, we’ll send you the new patient paperwork via email. Both the paperwork and the $125 new patient fee must be completed before we can schedule your first appointment.
New patient fee: $125.00
This fee is required regardless of insurance type.
It is not billable to insurance, and it does not count toward any deductible or copay.
This fee is separate from your visit and covers administrative onboarding, chart setup, and preparation for your initial consultation.

We are not in network with United Healthcare, Aetna Medicaid, or Tricare.
If your insurance plan is not listed, or if you are on a Medicaid plan not mentioned above, we are unfortunately not able to accept it at this time.
If you have a commercial insurance plan with out-of-network benefits or if your cash rate is lower than your in-network cost, we’re happy to discuss cash-pay options with you.
